iPhone does not allow websites to add the icon themselves. Use Safari Share.

Tap Share at the bottomThe square Share arrow is in the Safari bar at the bottom of the screen. This card stays out of the way so you can tap it.
Scroll, then Add to Home ScreenIn the Share sheet, scroll down until you see Add to Home Screen, then tap it.
Tap AddIf you see Open as Web App, turn it on, then tap Add. GET4CARD appears on your Home Screen.
